Consent Form for Children
For the vaccination of children under the age of 18, a signed consent form is required in which all legal guardians confirm that they consent to the vaccination. In cases of shared custody, both guardians must sign the form.The form mainly applies when a new vaccination series is initiated, but not for dose 2 or 3 within the same series.
When Is a Consent Form Required?
The form is always required, including for booster doses, if:
- An older child comes alone for vaccination.
- The child is accompanied by an adult other than their legal guardians.
