The health declaration helps us identify any potential obstacles to vaccination and makes your visit smoother. You can complete it digitally in advance or on-site.

Why fill out a health declaration?
The health declaration is designed to identify any contraindications to the planned vaccine, such as an allergy to an ingredient. If risks are present, we do not recommend vaccination.
How to Fill Out the Health Declaration
You can complete it in the way that suits you best:
On My Pages via our website
In our app (App Store & Google Play)
On-site using customer computers/iPads or QR codes available at the clinics
When You Arrive
Check in via the app or My Pages by clicking “Check In.”
On the Tick Bus, you will receive a queue number – remember to keep track of it.
At the clinic, you will be called when it’s your turn.
